Liability Waiver & Media Release

1. Assumption of Risk I, the undersigned, understand that basketball involves physical activity and risks of injury. I voluntarily assume all risks associated with my child’s participation in the Chicago Skills Lab clinics, including but not limited to falls, contact with other participants, and equipment usage.

2. Medical Release In the event of an emergency, I authorize Chicago Skills Lab staff to seek medical treatment for my child if I cannot be reached. I confirm that my child is physically fit to participate in strenuous athletic activity.

3. Media & Photography Release I grant Chicago Skills Lab the right to take photographs and video recordings of my child during clinics. I agree that Chicago Skills Lab may use these materials for marketing, social media (Instagram, website), and promotional purposes without further compensation or approval.

4. Liability Release I hereby release and hold harmless Chicago Skills Lab, its coaches, and the host school facility from any and all liability, claims, or causes of action arising out of my child’s participation in these programs.

By checking the 'I Agree' box on the registration form, I am providing my digital signature and agreeing to all terms listed above.
